In order to participate in Youth Ministry and Parish Events, each participating child and adult volunteer must fill out the required forms. These forms may be printed directly off the corresponding links, and turned into the parish. Both forms are valid for the dates of September 2016 to May 2017, and once these forms have been returned they will not have to be turned in again for the 2016-2017 school year.

 

Any adult willing to participate as a chaperone and driver is also required to fill out automobile information sheet and return it before the day of the event.
